If you have any questions, please contact your provider's office.

      3. If the information is correct, click the “Save” button.
      4. Once you have completed the clipboard, you will have an opportunity to review all your answers. You may modify by clicking Edit if necessary.
      5. Once you have completed your review, click “Submit Answers.”
    5. If I submit a clipboard, does it become part of my medical record? Once you fill out and submit a clipboard, expect your provider’s staff to confirm that nothing has changed. The information will become a part of your health information record.
