What is My IU Health?
My IU Health is an easy-to-use service and is the primary point of access for online patient services at IU Health. With My IU Health, patients can be a partner in their healthcare. It is an Internet-based hosted application that enables patients to easily and securely have access to a portion of their electronic health records as well as certain medical bills. Use of this online service is optional and no fee is charged to use it.
The services available to IU Health patients through the My IU Health portal include:
- Request or cancel appointments.
- View portions of your medical record.
- Receive select lab results.
- Communicate with certain IU Health providers via online messaging.
- Request prescription refills.
- Access patient education materials.
- View and pay IU Health bills, ask billing questions, request itemized statements and apply for financial assistance.
Users Access and Appropriate Use
My IU Health is provided on https://MyIUHealth.org as a source of information to support and not to replace the relationship that exists between you and your healthcare providers. You must register on My IU Health under your own name and keep your contact information in your My IU Health account up to date.
By accessing, browsing or using My IU Health, you represent and agree that you are at least eighteen (18) years old or have proper adult permission for an account as an adolescent between the ages of 14-17.
Pursuant to 45 CFR 164.524(a)(1),(c)(2)(ii), (c)(3)(ii), you may instruct IU Health, as a Covered Entity, to direct your ePHI to a third-party app. Pursuant to guidance from the Department of Health and Human Services, IU Health is not responsible for unauthorized access to your ePHI while in transmission to a third-party app at your direction. You, not IU Health, are responsible for the security of your health information once you direct IU Health to send it. You do so at your own risk. By using My IU Health, you agree to provide true, accurate, current and complete information about yourself. If you provide information that is untrue, inaccurate, not current or incomplete, or IU Health has reasonable grounds to suspect that such information is untrue, inaccurate, not current or incomplete, then IU Health has the right to suspend or terminate your account and refuse any or all current or future use of the My IU Health services.
You may disenroll from a My IU Health account at any time by calling Health Information Management, Monday-Friday, 8am-5pm, at 317.963.1661 or online at any time using your My IU Health account settings.
Access to My IU Health is granted for your convenience and to share information related to your healthcare. Your use of the system unrelated to this purpose may result in the suspension or termination of your access privileges. Improper activities include, but are not limited to, the use of inappropriate or abusive language, reserving appointment times that are not needed or are frequently canceled, and other activities that are deemed in IU Health's sole discretion to cause a disruption to clinical operations or are otherwise inappropriate uses of the system. Use of this site is subject to IU Health's general website Terms and Conditions which are incorporated into these My IU Health Terms and Conditions (to the extent that any of those general terms conflict with those listed here, the My IU Health Terms and Conditions shall govern).
Account Information and Security
If you initially enroll in person for My IU Health, you will be provided with an activation code (once you have created your personal login, the activation code will no longer be valid); otherwise, you may self-register online. The activation code allows you to create a personal login with a username and password that you create.
The username and password that you chose are your confidential credentials allowing you access to My IU Health - keep them secret. Anyone with access to your username and password are able to access your confidential medical information. That person is also able to read your messages and send communications as if he/she were you. No one at IU Health will know your password and it is up to you to keep it confidential. If at any time you feel that your security has been compromised, it is your responsibility to change your password. You can change your password online on the My IU Health website at any time.
As a registered user, you are permitted to give proxy access to another adult. If you agree to be a proxy or are offered proxy access, you must also agree to abide by these My IU Health Terms and Conditions. A My IU Health user may request proxy access for another individual at the patient's IU Health clinical office or online through the My IU Health portal. The parent or legal guardian of a child who is younger than 14 years old may have proxy access to the minor's My IU Health account; such access will only be granted to the extent that the individual given proxy access can demonstrate the legal right to the account holder's medical information. Proxy access by parents and legal guardians of minors is limited after the minor patient turns 14 years old. If the proxy's legal relationship with the account holder changes, then the proxy holder or account holder must notify IU Health by calling IU Health's Health Information Management at 317.963.1661. IU Health reserves the right to revoke proxy access at any time for any reason.
Health Record Ownership
The information provided through My IU Health is the sole property of IU Health and as an IU Health patient, you have a right to this information.
The medical records contained in My IU Health are confidential patient information. IU Health will, at all times, use reasonable efforts to maintain the confidentiality of your medical information in accordance with applicable rules and regulations. In consideration of your agreement to these My IU Health Terms and Conditions and for other valuable consideration, IU Health grants to you a nonexclusive, nontransferable, limited, terminable license to access and use My IU Health only in the United States. IU Health reserves all of its rights not expressly granted in these Terms and Conditions.
Indemnification and Hold Harmless
You agree to indemnify, defend and hold harmless IU Health, its officers, directors, employees, agents, suppliers and licensors (collectively the "My IU Health Providers") from and against all losses, expenses, damages and costs, including reasonable attorneys' fees, resulting from any violation of these My IU Health Terms and Conditions or any activity related to your My IU Health account (including negligent or wrongful conduct) by you or any other person accessing My IU Health using your My IU Health user credentials. This provision is for the benefit of IU Health and its officers, directors, employees, agents, suppliers and licensors. Each of these individuals or entities shall have the right to assert and enforce those provisions directly against you on its own behalf.
YOU EXPRESSLY UNDERSTAND AND AGREE THAT IU HEALTH AND ITS SUBSIDIARIES, AFFILIATES, OFFICERS, EMPLOYEES AND AGENTS SHALL NOT BE LIABLE TO YOU FOR ANY PUNITIVE, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL OR EXEMPLARY DAMAGES, INCLUDING BUT NOT LIMITED TO, DAMAGES FOR LOSS OF DATA OR OTHER INTANGIBLE LOSSESS (EVEN IF IU HEALTH HAS BEEN ADVISED OF THE POSSIBLIITY OF SUCH DAMAGES), RESULTING FROM: (i) THE USE OR THE INABILITY TO USE MY IU HEALTH; (ii) UNAUTHORIZED ACCESS TO OR ALTERATION OF YOUR TRANSMISSIONS OF DATA THROUGH THE MY IU HEALTH SYSTEM DUE TO NO FAULT OF IU HEALTH; OR (iii) STATEMENTS MADE BY, CONDUCT OF, OR ANY INFORMATION, SERVICES OR GOODS PROVIDED BY A THIRD PARTY.
These My IU Health Terms and Conditions shall govern and be construed in accordance with the laws of the State of Indiana. You agree that any legal action or proceeding between IU Health and you for any purpose concerning your use of My IU Health or the parties' obligations hereunder shall be brought exclusively in a court of competent jurisdiction sitting in Marion County, Ind. Any cause of action or claim you may have with respect to My IU Health must be commenced within one (1) year after the claim or cause of action arises. IU Health's failure to insist upon or enforce strict performance of any provision of these My IU Health Terms and Conditions shall not be construed as a waiver of any provision or right.
You are always welcome to use certain IU Health websites without registering or providing any personal information. However, registration is required for certain services, such as discussion boards (when available) and other features, including My IU Health. When you use these features, we will clearly disclose what information is required, what information is optional and how that information may be used.
